Thank you for choosing CIMI to enhance your students' education. They will benefit from an unforgettable, exciting, hands-on, fun learning experience. The forms and handbook contained below can all be downloaded and viewed using Adobe Acrobat. If you do not have Acrobat, please download it here before opening the files. Some of the forms need to be filled out and returned to us prior to your arrival at Fox Landing.

The Teachers Handbook, which includes forms to be sent back to us. The handbook also gives detailed descriptions of how camp will run and should answer any questions you, other chaperones or parents might have. Please note the checklist inside the front cover to keep your organized. (Some of the forms found below)
3-Day Program Planning form (Green).
5-Day Program Planning form (Blue).
Classroom Preparation form (Yellow).
Pre-Trip Logistics sheet which will give you detailed instructions on what to do before leaving for CIMI.
Telephone Contact sheet (Pink). Please fill this out but DO NOT mail it back to us as you will be using it.
Adult Waiver form (Orange). All adults attending must fill this form out and sign it.
Student Medical Form. These need to be filled out and taken with you to camp. DO NOT mail to us. Student Medical Form (Spanish). These need to be filled out and taken with you to camp. DO NOT mail to us.
Approximately two weeks prior to your trip, our program coordinator will call to go over details; this is when you will want to have the telephone contact page on hand. All of the information you provide will let us know how we can best meet your needs.
The island office is available to make your trip preparations run as smoothly as possible. Feel free to call on us should you have any questions, concerns or ideas. We want to do everything we can to help!
